1. Introduction
Thank you for joining the Xray Dose Company dosimeter badge service. By enrolling in and using our radiation monitoring services, you agree to comply with and be bound by the following terms and conditions, which govern your use of our service. These terms outline your responsibilities, our service commitments, and important information regarding billing, reporting, and compliance. Please review them carefully before subscribing, as continued use of our service constitutes acceptance of these terms.
2. Description of Services
The dosimeter badge service provided by Xray Dose Company includes the ongoing issuance, required wearing, analysis, and reporting of personal radiation dosimeter badges on an annual basis. This service is designed to accurately measure and document occupational radiation exposure for individuals working in environments where ionizing radiation may be present. Through regular monitoring and detailed exposure reports, the service helps ensure radiation levels remain within applicable safety limits and supports compliance with regulatory and workplace safety requirements.
3. Subscription and Fees
3.1 Service Fees:
Xray Dose Company’s dosimeter badge service is billed at an annual, per-person rate. This fee includes the dosimeter badges, routine processing, exposure analysis, reporting, and standard shipping. All fees are non-refundable unless otherwise expressly stated in writing by Xray Dose Company.
3.2 Late or Unreturned Badge Fees:
Dosimeter badges must be returned according to the agreed-upon exchange schedule (monthly or quarterly) to ensure accurate monitoring of radiation exposure and timely review of results. If a badge is not returned within sixty (60) days following the end of the designated wear period, a late or unreturned badge fee of sixty dollars ($60) per badge will be charged.
3.3 Lost or Damaged Badge Fees:
If a dosimeter badge is not returned at all, or if a badge is returned in a damaged or unusable condition—even if returned within the sixty (60) day timeframe—a lost or damaged badge fee of sixty dollars ($60) per badge will be charged to cover replacement and processing costs.
3.4 Payment Terms:
Payment is due and becomes effective at the time an order is placed with Xray Dose Company. Unless otherwise expressly agreed to in writing by both Xray Dose Company and the client, payment for annual service must be made in full at the time of purchase.
3.5 Subscription and Renewal:
By subscribing to the dosimeter badge service, you agree to pay the applicable subscription fees as outlined in your service agreement. Subscriptions are provided on an annual basis and will automatically renew at the end of each yearly term unless cancelled in accordance with Xray Dose Company’s cancellation policy.
4. Dosimeter Badge Usage
4.1 Proper Use:
Subscribers agree to use dosimeter badges solely for their intended purpose of monitoring radiation exposure. Badges must be worn, stored, and handled in accordance with provided instructions. Any misuse, unauthorized handling, tampering, damage, or alteration of a badge is strictly prohibited, as such actions may compromise the accuracy of exposure data and regulatory compliance.
4.2 Badge Assignment and Transfer Restrictions:
Dosimeter badges are assigned to specific individuals or designated monitoring locations and are strictly person- and area-specific. Badges may not be shared, transferred between individuals, or moved from one physical location to another. If an employee, student, monitored area, or badge wearer relocates to a different facility or site, a new badge must be ordered to ensure accurate exposure tracking and proper recordkeeping.
4.3 Badge Return Requirements:
All dosimeter badges must be returned at the end of each designated wear period to the return address specified in the materials provided to the client. Timely return is essential for accurate dose analysis and reporting. Any badge not returned within the required timeframe will be considered unreturned and will incur a replacement fee of sixty dollars ($60) per badge.
5. Monitoring and Reporting
5.1 Radiation Monitoring:
Radiation exposure is measured through the use of assigned dosimeter badges worn during the designated monitoring period. These badges record occupational exposure to ionizing radiation, and the collected data is analyzed to generate exposure results. Standard reports are provided on a quarterly basis unless otherwise specified in the service agreement.
5.2 Exposure Reports:
Radiation exposure reports are delivered to the subscriber according to the reporting schedule agreed upon at the time of enrollment. Reports include individual and, where applicable, area-based dose information and are intended to support regulatory compliance, safety oversight, and internal record-keeping.
6. Responsibilities of Subscriber
6.1 Regulatory Compliance:
Subscribers are responsible for understanding and complying with all applicable federal, state, and local regulations, standards, and guidelines governing radiation exposure within their industry. Xray Dose Company provides monitoring and reporting services to support compliance; however, ultimate responsibility for adherence to regulatory requirements, workplace policies, and corrective actions remains with the subscriber.
6.2 Accuracy of Information and Proper Use:
Subscribers must provide complete and accurate information regarding all individuals or locations assigned dosimeter badges, including names, roles, and monitoring requirements. Subscribers are also responsible for ensuring that badges are worn correctly and consistently during the designated wear period, as improper use or inaccurate information may affect the validity of exposure data and reporting.
7. Liability
7.1 Limitation of Liability:
Xray Dose Company shall not be held liable for any direct, indirect, incidental, consequential, or special damages, losses, or claims arising from the improper use, mishandling, or misuse of dosimeter badges, or from a subscriber’s failure to follow safety procedures, wearing instructions, or applicable regulatory requirements. Xray Dose Company’s services are intended to support radiation monitoring and compliance efforts, but do not replace the subscriber’s responsibility to maintain a safe working environment and comply with all applicable laws and guidelines.
7.2 Indemnification:
Subscribers agree to defend, indemnify, and hold harmless Xray Dose Company, its officers, employees, and affiliates from and against any and all claims, damages, liabilities, losses, costs, or expenses (including reasonable attorneys’ fees) arising out of or related to the subscriber’s use of the dosimeter badge service, including but not limited to improper use of badges, inaccurate information provided by the subscriber, or failure to comply with applicable safety or regulatory requirements.
8. Termination
8.1 Termination by Subscriber:
Subscribers may terminate their dosimeter badge service by providing at least forty-five (45) days’ written notice to Xray Dose Company. Fees paid for the current billing cycle are non-refundable. To discontinue monitoring for a specific individual, area, or badge, subscribers must notify Xray Dose Company in writing via email to request badge inactivation. All inactive or cancelled badges must be returned within sixty (60) days following the end of the applicable wear period to avoid unreturned badge fees.
8.2 Termination by Xray Dose Company:
Xray Dose Company reserves the right to suspend or terminate service, with or without notice, for any subscriber who fails to comply with these Terms and Conditions, including but not limited to non-payment, misuse of dosimeter badges, or failure to meet return and reporting requirements.
9. Amendments
Xray Dose Company reserves the right to modify, update, or amend these Terms and Conditions at any time to reflect changes in services, regulations, or business practices. Subscribers will be notified of any material changes via email and/or other communication means. Continued use of the dosimeter badge service after such notification constitutes acceptance of the revised Terms and Conditions.
10. Governing Law
These Terms and Conditions are governed in accordance with the standards and requirements set forth by the National Voluntary Laboratory Accreditation Program (NVLAP), which accredits Xray Dose Company's contracted laboratory for dosimetry services. Compliance with NVLAP ensures that all monitoring, reporting, and operational procedures meet recognized national quality and accuracy standards. All dosimetry practices follow the ALARA (As Low As Reasonably Achievable) principle, ensuring radiation exposure is minimized while maintaining necessary operational functions. Subscribers agree that any disputes or questions regarding these Terms and Conditions will be interpreted and resolved with reference to NVLAP guidelines, ALARA principles, and accreditation requirements.
